Staff RN - POSTPARTUM Fontana, CA. As the leader of the health care team, you will provide professional nursing care, utilizing the nursing process in accordance with established standards of care, policies, and procedures. You will demonstrate performance consistent with the Medical Center Vision, Mission, and Strategic Plan of the organization, remain flexible to changing systems, and treat co‑workers, patients, families, and all members of the health care team with dignity and respect. Essential Responsibilities Leadership Uphold Kaiser Permanente policies and procedures, principles of responsibilities, and applicable state, federal and local laws. Serve as a leader of the health care team; delegate tasks appropriately, and demonstrate appropriate accountability. Understand own and team members’ scopes of practice and escalates issues as appropriate. Demonstrate professional, supportive behavior; champion new ideas. Lead and direct others through the change process. Utilize communication strategies including chain of command and issue escalation to achieve intended outcomes. Participate in problem identification and resolution. Mentor, orient, and coach others in unit specific operations and patient care activities. Share responsibility and authority with subordinates and hold them accountable for performance. Problem‑solve with other departments to assist member problem resolution. Prioritize, delegate, and supervise work assignments to ensure completion of patient care activities. Comply with regulatory requirements, policies, procedures, and standards of practice. Nursing Process Develop and/or contribute to individualized plans of care that reflect assessment, planning, implementation, and evaluation of outcomes. Ensure plans show multidisciplinary planning, consultation, and education. Ensure plans are reflective of admission or outpatient databases, ongoing findings, age‑appropriate care, cultural needs, and appropriate acuity. Ensure plans are discussed with patient, family/significant others, and completed in a timely manner. Monitor patient progress based on the plan and revise on an ongoing basis. Ensure care meets standards of practice and that effective discharge plans are completed and understood by the patient and family. Ensure patient safety, including medication administration, falls prevention, pressure‑ulcer prevention, and infection control. Ensure optimal pain control and patient comfort; address patient anxieties, fears, or concerns. Ensure patient understands medication purpose, side effects, and administration instructions. Documentation Chart accurately, legibly, and in a timely manner. Document nursing interventions, evaluations, and assessments. Utilize computer systems efficiently for optimal patient care. Clinical Outcomes Discuss patient findings and progress toward outcomes with physicians and other team members. Make comprehensive nursing decisions based on data, assessments, and evaluations. Participate in departmental performance improvement activities. Workplace Safety Adhere to LMP Workplace Safety principles and practices. Apply standard precautions and maintain a safe environment for self and others. Patient Care Experience Practice customer service standards as defined by the Service Area, Medical Center, and department. Answer call lights, alarms, and patient requests promptly. Make referrals and facilitate patient use of resources. Maintain and protect patient confidentiality. Ensure a clean, orderly, and functional work environment. Treat families with courtesy, respect, kindness, and compassion. Actively listen to patient and family needs and meet those needs. Provide information in a way patients understand and confirm comprehension. Provide a patient experience that exceeds member expectations. Identify and assist in simplifying or correcting systems. Utilize payroll and non‑payroll resources efficiently. Basic Qualifications One year of recent (within the last 3 years) full‑time equivalent experience in mother/baby care or successful completion of a KP FCC training program or an approved equivalent within the prior 12 months. Registered Nurse License in California. Basic Life Support certification from the American Heart Association. Neonatal Resuscitation Program certification. Additional Requirements May be expected to cross‑train to at least one other Perinatal area. Shift Information If the 12‑hour option is chosen, the shift will be 7:00pm–7:30am; a backup 8‑hour shift will be 11:00pm–7:30am.
